The “Cruciform”®
Priority Cleaning System card has a number of pages, as follows:

PRIORITY 1
This
starts the cleaning/disinfection process and must be completed first.
Mostly tick boxes but has sections for cause of infection, infestation
or contamination, names of cleaning/disinfection operatives and
person giving authority to start process and ensure that preparatory
issues are dealt with. |

PRIORITY
2
This
page follows the priority 1 page and continues the cleaning/disinfection
process, with emphasis on removing gross contamination and infectedX
objects. |

|
PRIORITY
3
This
page follows the priority 2 page and continues the process with
emphasis on cleaning, then disinfection of all objects and surfaces
in the room/space. |

White
Page
This
page ensures that all foregoing processes have been satisfactorily
completed and that all administrative issues have been dealt with
and forms handed over.

Certificate
of Decontamination
This
page must be signed by the contractor’s representative before
handing it over as a link to the completed
record of cleaning / disinfection which will be securely stored by the contracting organisation. Once completed
its handed over to custodian/owner or representative as a numbered
record, which can be referred to in the future if required. It has
the unique identification number.

Contents
of Kit Page
This page identifies all contents of cleaning/disinfection kit,
the address of the space/room cleaned
and contains the same unique identification number as on the Certificate
of Decontamination.

Assessment Sheet
This
two-part form enables all risks present before the cleaning/disinfection
starts to be identified and agreed. Unique reference number on “Cruciform”®
card is filled in on this sheet to provide a link. |

Listing
Sheet
This
two-part form enables all items Damaged and Left, Bagged for Cleaning,
Bagged for Disposal, Personal Possessions handed over, Contaminated
and Disposed of and Items Returned to Contractor to be identified
and agreed. Unique reference number on “Cruciform”®
card is filled in on this sheet to provide a link. |

Hazardous
Waste Transfer Note
This
two-part form ensures that cleaning contractor and Licensed Waste
Disposal Company have an agreed record of all items/bags taken away
for appropriate disposal. Unique reference number on “Cruciform”®
card is filled in on this note to
provide a link.

The above
linked forms/sheets/certificate ensure that a complete, auditable and
verifiable record is produced for each contamination case. This protects
all concerned and ensures that any litigation or other legal claim made
can be adequately defended.
